Welcome to the Chippewa Falls Area Unified School District! In order to get your child registered as a new student into our district, we will need the following registration forms completed prior to the start of the 2012-2013 academic school year (Household Enrollment Form, Student Enrollment Form, Health Information Update Form and Student Immunization Record). You can click on the links below to open and print these registration forms. If you do not have access to the internet and/or a printer, please feel free to contact Lisa Gibson at the District Office (715) 726-2417 ext. 1803. You can either pick the forms up at the District Office, or Lisa can mail them to your home. Please note that if your child is new to our district and is entering into Kindergarten for the 2012-2013 academic school year (new student to our district that has not attended our BB4C program), a verification of a certified birth certificate will be required. The elementary school secretaries have the summer off and return to work on August 17, 2012. While they are out on summer break, please contact Lisa Gibson directly to register your child for the 2012-2013 school year. Back to school information will be sent out to families after the school secretaries return in August.
