Eligibility Checklist

• Activity Code Meeting- ALL students MUST attend a Competitive Activities Code meeting with a parent/legal guardian, AT LEAST ONCE, in their high school career.  If you are unable to attend the Code Meeting in person, you may watch it here: Chi-Hi Code Meeting

• Complete Online Registration each year.
• "WIAA Preparticipation Physical Form" on file at Chi-Hi prior to practice or participation.  Physical exams taken April and thereafter are valid for the following two school years; physical exams taken before April are valid only for the remainder of that school year and the following school year.
• Pay Activity Fee online through the Infinite Campus Parent Portal (Click here for instructions) or in the Main Office at Chi-Hi
